A greater understanding and uptake of implementation science frameworks and measures can help both cancer control practitioners and researchers leverage crucial insights into how to best deliver research-based initiatives in the complex communities where they are crucially needed. In April, the National Cancer Institute (NCI) released Implementation Science at a Glance, a workbook to introduce practitioners and policymakers to the building blocks of implementation science. This resource is a natural extension of our experiences funding [1] and training researchers in implementation science as well as perspectives NCI gained through this work [2].

Many advances have been made; for example, the number of people receiving antiretroviral drugs has increased five-fold in low- and middle-income countries between 2003 and 2006 [1]. However, major obstacles remain, including severe shortages of labour and infrastructure, which are the building blocks of comprehensive programs that will help achieve the MDG target. In a UNAIDS report [1], it is estimated that the number of full-time equivalents of health personnel needed to achieve the universal-access targets by 2010 are nearly half a million, though other sources place this estimate at twice that number [2]. Patient residences and hospital locations were recorded as census region which are in essence aggregated zip code areas. Switzerland is divided into 605 census regions; their precise name in the federal discharge dataset is 'MedStat-region'. These regions constituted the geographic building blocks of subsequent HSAs. Their creation accounted for demographic, socio-economic and geographic criteria to ensure uniformity and comparability. Two-hundred-and-fifteen census regions contained at least one hospital; we termed them hospital regions. Unfortunately, the Swiss hospital discharge master file does not contain zip codes for patient residence but only the respective MedStat-region. This limitation is attributable to strict confidentiality laws and is unlikely to change in the near future.


Figure 1 gives the reader an overview of Swiss topography. The massive Alps run along the entire south and cover roughly half of the country's area. The Jura mountain range runs along the northwest. In between stretch the Swiss midlands, from southwest to northeast. They are densely populated and contain most urban centres. Figure 2 shows the 605 census regions, which are the building blocks for our HSA. We wish to point out the large census regions in the Alps and the Jura range. In comparison, census regions are smaller and more numerous in the Swiss midlands. Urban centres like Basel, Bern, Geneva, Lausanne, and Zurich are clearly recognizable.
